I opened Premium Physical Therapy LLC in 2019 because I wanted a practice where every visit still feels like a conversation — not a conveyor belt. Here in Stone Mountain, we keep the caseload small so your evaluation, your exercises, and your next steps stay tailored.

Absolutely. Most new patients start with an evaluation and a gentle first plan. We meet you where you are.
We can bill many plans. The rates above are self-pay references — note your carrier on the form and we’ll confirm benefits.
Photo ID, insurance card if applicable, imaging or operative notes, and clothes you can move in.
Please give 24 hours’ notice when you can. Same-day no-shows may be charged a visit fee.
